Parts A and B are basically medical history and information.

Part C requires a DR visit.  Sports physicals will cover the information needed.

This form is required for any overnight camping event and is good for a year.

If you completed one to go to the Parent-N-Pal / Webelos Woods camp-out,
you may already be done.

These forms are required to check in to camp - they make no exceptions.

The form is "fill-able", so you can complete the form on your computer if you'd like before printing.
You can't save it, or update it later, so be sure to double check everything.
(However, on your computer you probably have a "printer" called Microsoft XPS Document Writer -
  select that and it will create a pdf like file that you can re-open/re-print later - basically saving it)
