Boy Scouts of America (Youth/Adult) Applications - These must be obtained at one of our meetings from the Committee Chair.

Pack 9 Welcome Packet - This is a packet we developed to help introduce Pack 9 and scouting to our parents and their new scouts.  

Annual Health Form - Each registered member (scouts and adults) must complete an Annual Health Form each year.  This form is available in English & Spanish.

Annual Health Form - Immunization Exemption - This exemption may be used by adults to claim "exemption" from documenting their shot record information (only) on the Annual Health Form.  

Motor Vehicle Form - A Motor Vehicle Form must be completed for each vehicle to be used during Pack activities, fieldtrips and campouts.  Fill out a form for each vehicle.
