Scout Application



Every new scout is required to complete an application and turn it in to one of the adult leaders in the troop.  You can find one at the link above, attached below, or by contacting the Scoutmaster by clicking on Contact Our Troop.

Adult Leader Applications



Troop 717 welcomes adult volunteers to help develop out scouts.  Active parent participation is essential to ensure the success of the pack, but it isn't necessary for you have a scout in the troop to volunteer to help.  Some of the positions include:
- Scoutmaster
- Assistant Scoutmaster
- Troop Committee Chair
- Training Chair
- Membership Chair
- Treasurer
- Equipment Coordinator
- Outdoor/Activities Chair
- Advancement Chair

Adults interested in helping the troop should discuss their interests with the Scoutmaster or Troop Committee Chair by clicking the Contact Our Troop link above.  All adult leaders must complete an application (available below).  Registration and training fees are paid by Troop 717.  In addition, if you are interested in being a merit badge counselor, fill out the Merit Badge Counselor Application (also available below).  You can find a current list of merit badges and other resources for merit badge counselors at this link: http://www.scouting.org/scoutsource/BoyScouts/GuideforMeritBadgeCounselors.aspx.

Membership Transfer Form



If you are already registered in Scouting, either as a Scout or as an adult volunteer, and you are transfering to Troop 717, please fill out the form below so that we can transfer your membership to our troop with a minimum of hassle.

Annual Health and Medical Record



The BSA Annual Health and Medical Record may be require for participants of certain activities.  All parts are included in the attachment below.

Parts A and B are required are to be completed at least annually by participants in all Scouting events. This health history, parental/guardian informed consent and release agreement, and talent release statement are to be completed by the participant and parents/guardians.

Part C is the physical exam that is required for participants in any event that exceeds 72 consecutive hours, for all high-adventure base participants, or when the nature of the activity is strenuous and demanding. Service projects or work weekends may fit this description. Part C is to be completed and signed by a certified and licensed health-care provider—physician (MD or DO), nurse practitioner, or physician assistant. It is important to note that the height/weight limits must be strictly adhered to when the event will take the unit more than 30 minutes away from an emergency vehicle, accessible roadway, or when the program requires it, such as backpacking trips, high-adventure activities, and conservation projects in remote areas.

Part D is required to be reviewed by all participants of a high-adventure program at one of the national high-adventure bases, and shared with the examining health-care provider before completing Part C.  The national high-adventure bases are:
- Philmont Scout Ranch
- Northern Tier National High Adventure Program
- Florida National High Adventure Sea Base (Requires PADI Medical Form if scuba diving will be conducted, attached below)
- Summit Bechtel Reserve
