Below is a listing of forms needed to carry out your role as a Troop/Group Leader. 

Authorization Agreement for Direct Payment – Use this form to set your account for ACH for product sales. 

Trip Approval/Information Form – This form is used prior to taking a trip outside of the council jurisdiction. 

Parent Permission Form  – Use this form any time your troop leaves their troop meeting place. Remember that sensitive issues, high risk activities and product sales require separate permission slips.

Troop Budget Worksheet   - This form is due to the council at the end of each school year. This form provides an update on individual troop finances
