Before coming to the first troop meeting, please download and print the form (PDF file is attached below).  Fill in the information and give the completed form to the Scoutmaster or Assistant Scoutmaster. 

Annual Health and Medical Record
Before coming to the first troop meeting, please download the form (PDF file is attached below).  This form needs to be completed by every registered boy scout and any registered adult who will be joining in on scouting activities lasting more than 72 consecutive hours.  Type in the information and print the form.  Part B must be completed and signed by a certified and licensed health-care provider—physician, nurse practitioner, or physician’s assistant.  Once you have the form completely filled out and signed by the health-care provider, please give the form to the Scoutmaster or Assistant Scoutmaster.

Activity Consent Form
Before going on a troop activity, this form needs to be completed (PDF file is attached below).  Type in the information and print the form.  Give the completed form to the Scoutmaster or Assistant Scoutmaster.

Mandatory Training

Effective June 1, 2010

• Youth Protection training is required for all BSA registered volunteers, regardless of their position.
• New leaders are required to take Youth Protection training before submitting an application for registration. The certificate of completion for this training must be submitted at the time the application is made and before volunteer service with youth begins.
• Youth Protection training must be taken every two years. If a volunteer’s Youth Protection training record is not current at the time of recharter, the volunteer will not be reregistered.
