Cub Scouts and parents who with to chaperone Scouting activities such as day camps or summer camping trips must have an annual medial evaluation. In most cases only Parts A and B of the Annual Health and Medical Record must be filled out. Some Cub Scout activities might require this Activity Consent Form. Parents will be notified by the Den leaders when it is necessary to sign and return this form so that your Cub Scout can participate in an Pack activity.
