Please fill out and return the following to your den leader:

1.  Family Talent Survey Sheet
2.  Annual Health  (form A) and Medical Record
3.  Request for exemption from medical care and treatment (if applicable)
4.  Complete the Youth Protection web based training and return completion certificate to Den Leader.

     Youth Protection( requirement for every adult) short cut to website.  You must register for user ID and password.  
     Use this site for training too.
